As a responsible dog rescue charity, we take rehoming extremely seriously. Please complete our Adoption Enquiry Form thoroughly and in as much detail as possible. This will increase the likelihood of your application being accepted, and also enable us to match you with a suitable dog.

I have completed this form thoroughly and in as much detail as I can. I understand that Street Hearts BG take rehoming dogs very seriously so, if my application form lacks sufficient information, it could be rejected *

I understand that the submission of this form is the first stage of the Street Hearts BG adoption process. If shortlisted, I will be required to attend a video interview where I will meet the dog I am interested in, and a home check will be conducted *

I understand that Street Hearts BG charge a non-refundable adoption fee of £550 per dog (reduced to £450 for dogs over seven years of age) *

I understand that, if my application is successful, I will be required to collect my dog from a designated collection point. Street Hearts BG will endeavour to make the collection point as convenient as possible; however, I am aware that I will be expected to travel in order to collect my adopted dog. *
